Carrier bill reconciliation
Carrier bill reconciliation is the recurring control that compares an insurance carrier’s premium invoice against your enrollment and payroll records before you pay it.
The comparison runs at member level, matching each covered employee and dependent on plan, coverage tier, effective dates, and premium. Reconcile every billing cycle, which for most group plans means monthly. The enrollment side reaches the carrier through the carrier feed: the recurring transmission of new elections, tier changes, dependent updates, and terminations. A sent record is not an accepted one, so read the rejection report after each run. Waiting a quarter multiplies the work and can push credits past the window a carrier allows for retroactive adjustments.
How does a member-level reconciliation run?
A total premium match is not enough. Take one employee billed at family who elected employee only, and another billed the reverse. The total reconciles while both records are wrong. Member-level matching finds those errors in the month they appear, when a credit is still routine to request. Every mismatch category needs a default owner, a response target, and the evidence required to close it.
- 01
Identify the changes first. Additions, terminations, retroactive changes, and unmatched records.
- 02
Separate expected timing differences from true exceptions: a monthly invoice and a per-payroll deduction reconcile over the coverage period, not on every pay date.
- 03
Sort the remaining mismatches into categories: eligibility, plan or tier, effective date, identity, premium, missing carrier transaction, missing payroll deduction, termination, and arrears.
- 04
Record the approved payable amount alongside the credits or adjustments requested, which makes the following month’s review fast.
